Student and Parent/Guardian Concussion Code of Conduct Forms
Hilson prides itself on running a dynamic extra-curricular athletics program.
To participate, students grades 3 to 6 must have 2 forms completed:
1) Parent/Guardian Concussion Code of Conduct Form
2) Student Code of Conduct Form
Please fill out these forms using the links above at your earliest convenience. Once the forms are completed your child will be able to participate in any and all intramural sports that interest them for the duration of the school year.

Appropriate Use of Technology Agreement
Parents/guardians of new OCDSB students and students at the beginning of grades 4, 7 and 9 are asked to review with their children the Appropriate Use of Technology Agreement and submit this agreement. This is a key part of ensuring your children’s safety when using technology. Your home discussion about the appropriate use of technology also supports our educators’ discussions about this topic at school.
Media/School Website Permission Form
The Municipal Freedom of Information and Protection of Privacy Act (MFIPPA) requires us to obtain permission from parents/guardians to publicize information that may identify their children. Parents/Guardians are asked to choose whether or not we can include their child's work, photos, and videos on communication platforms, as described below, when showcasing their talents and participation, or when publicizing school events and activities. If you choose "Consent is not granted", your child's picture will still appear in the school yearbook, if the school publishes a yearbook. Please complete the Media/School Website Permission Form, one per each child, to indicate whether or not you give permission to publicize information that may identify your child(ren).
